In order for them to patch codes on MW3, they have to know the code's, yes or no (I really have no idea.)
If so, that mean's that most likely there is someone from IW or Treyarch on this site. Discuss.

no sir they do not :P patching things means taking or putting stuff into the game making addresses shift around. so the codes shift around. they did not patch codes specifically. just added things into the game. for example making the damage on the striker more balanced and it might have messed up a couple of codes.

You'll probably get a better answer from a coder than me, but from what I know when there's a update bytes move around, meaning that codes can be off by a few bytes and will no longer work.
If there's a site that could potentially ruin a companies products or reputation, of course they'll keep a eye on it.
I'm sure a few game companies keep eyes on http://geckocodes.org/ to know what to update.

Codes are "patched" unintentionally most of the time. Updates just rearranges values and what addresses are being read to create room for more information. If there is a small update some codes survive because that area in the ram wasn't changed at all. Codes aren't "patched" because they can all be remade one way or another unless the company completely wipes that part of ram. Codes aren't "patched" they are just moved.
